This broadcast profile service is operated by USA Sport AZ LLC. This agreement covers the submitted rider profile and media only. It does not register a rider for an event, authorize participation, waive injury claims, or replace any organizer’s separate event waiver or parent/guardian participation consent.

The adult rider, or the parent or legal guardian signing for a rider under 18, grants USA Sport AZ LLC and production providers acting on its behalf a nonexclusive, worldwide, royalty-free license to reproduce, crop, remove backgrounds from, format, display, distribute, and publish the submitted rider name, photograph, biography, hometown, race details, social handle, and sponsor information in InsiderMX or InsiderMTB broadcasts, recordings, websites, social channels, and promotion of that coverage. You retain ownership. This permission does not authorize unrelated product endorsements, sale of your likeness, misleading edits, or AI model training.

Authorized coverage may remain available in archives. No payment, royalties, final approval, broadcast placement, sponsorship, or particular exposure is promised. Sponsor names identify the rider’s stated affiliations and do not establish an endorsement by the rider or sponsor of any unrelated product. You confirm the information is accurate and that you own the photo or have the photographer’s permission to grant these rights. Do not submit another person’s private information or material you lack permission to use.

To the extent permitted by law, the signer releases USA Sport AZ LLC and its providers from privacy and publicity claims arising solely from uses expressly authorized here. This release does not cover unauthorized uses, fraud, willful misconduct, gross negligence, or rights that cannot lawfully be waived. A parent’s signature does not purport to waive a child’s nonwaivable rights. If a provision is unenforceable, the remaining provisions continue to the extent permitted by law.

For a rider under 18, the parent or legal guardian must complete and sign this form personally, provide their own contact details, and confirm authority to make these decisions. A coach, team manager, photographer, sibling, or other accompanying adult may not sign unless they are the rider’s legal guardian. Riders under 13 may have profiles only with parent supervision: the parent or legal guardian must personally provide all details and photos and sign the release for the child. A child must hand the form to their parent or legal guardian before any upload or personal details are entered. The adult uses their own email and phone. These statements record the adult’s attestation; they are not independent verification of identity or parental authority.

Your typed full legal name is your electronic signature. By checking the agreement box and submitting, you intend to sign this release electronically and agree to use electronic records for this submission. The release version, accepted text, signer role, acknowledgments, and server receipt time are saved with the profile. You may print or save this page before signing; if you do not want to sign electronically, contact USA Sport AZ LLC by mail for another arrangement.

Profile privacy notice: rider details and photos are used for production and the public media uses described above. Email, phone, signatures, and guardian details are for administration and consent records, not on-screen graphics. Photos are sent to remove.bg for background removal when selected, before final submission; submitted profiles and processed photos are stored using Cloudflare D1 and R2 and can be downloaded by authorized production staff. The site also loads Google Analytics for site usage measurement. Public coverage can be copied by viewers and social platforms. Avoid sensitive details in biographies. This notice covers this intake, not the independent practices of linked sites or social platforms.

Write to USA Sport AZ LLC, 4539 N 22nd St, #7616, Phoenix, AZ 85016, to request access, correction, deletion, or withdrawal of permission for future use. Identify the rider and event; we may need to verify your authority. Withdrawal applies prospectively after processing and does not undo prior authorized publications or copies outside our control. We may retain consent records as needed to document prior permission or comply with legal obligations. Material changes to the authorized scope require new consent.
